fix: correct codeql workflow permissions, cron, and action ref

- Set root-level permissions to {}
- Add job-level permissions (actions, contents, packages, security-events)
- Pin action ref to commit hash with version comment
- Fix mangled cron schedule
- Clean up workflow structure
